1. What SageAlly does

SageAlly provides care navigation and care coordination services, including Medicare-covered navigation and care management programs delivered on behalf of physician practices. The specific services you or your practice receive are governed by a separate written agreement, enrollment form, or business associate agreement. If those documents conflict with these Terms, those documents control.

2. Not medical, legal, or financial advice

Content on the Site, including articles, guides, and educational materials, is for general information only. It is not medical, legal, insurance, or financial advice and does not create a provider-patient or attorney-client relationship. Always consult your physician or another qualified professional about your specific situation. SageAlly does not provide emergency services. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. If you are in crisis, call or text 988.

3. Insurance and Medicare coverage

Statements that services "may be covered" by Medicare or other insurance are general. Coverage depends on eligibility, plan, physician order, and payer rules. We do not guarantee that any service will be covered or reimbursed, and patients may be responsible for applicable copays, coinsurance, or deductibles as billed by their provider. SageAlly is not affiliated with or endorsed by Medicare or any government agency.

4. Eligibility

You must be at least 18 years old to use the Site. If you submit information about another person, you confirm that you are authorized to do so.

5. Information you submit

You agree that the information you provide is accurate. Please do not send patient health information through general contact forms or unsecured email; we will provide a secure way to share it once we are working with you. Our handling of information is described in our Privacy Policy and Notice of Privacy Practices.

6. Communications

By providing your phone number or email, you agree that we may contact you about your inquiry or services. If you opt in to text messages, messages may be sent by automated means; message and data rates may apply; reply STOP to opt out or HELP for help. Consent to marketing messages is not a condition of receiving services.

7. Acceptable use

You agree not to: use the Site for any unlawful purpose; attempt to gain unauthorized access to the Site or our systems; interfere with the Site's operation; scrape or copy Site content in bulk; or impersonate another person.

8. Intellectual property

The Site and its content, including text, graphics, logos, and the SageAlly name, are owned by or licensed to Sage Ally LLC and protected by law. You may view and print content for personal, non-commercial use. Any other use requires our written permission.

9. Third-party links

The Site may link to websites we do not control. We are not responsible for their content or practices.

10. Disclaimers

THE SITE IS PROVIDED "AS IS" AND "AS AVAILABLE." TO THE FULLEST EXTENT PERMITTED BY LAW, WE DISCLAIM ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, AND NON-INFRINGEMENT. WE DO NOT WARRANT THAT THE SITE WILL BE ERROR-FREE OR UNINTERRUPTED.

11. Limitation of liability

TO THE FULLEST EXTENT PERMITTED BY LAW, SAGE ALLY LLC AND ITS OWNERS, EMPLOYEES, AND CONTRACTORS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES ARISING FROM YOUR USE OF THE SITE. OUR TOTAL LIABILITY FOR ANY CLAIM RELATED TO THE SITE WILL NOT EXCEED ONE HUNDRED DOLLARS ($100). This section does not limit liability that cannot be limited by law and does not govern services provided under a separate agreement.

12. Indemnification

You agree to indemnify and hold harmless Sage Ally LLC from claims arising from your misuse of the Site or violation of these Terms.

13. Governing law

These Terms are governed by the laws of the State of New Jersey, without regard to conflict-of-law rules. Any dispute will be brought in the state or federal courts located in Ocean County, New Jersey.

14. Changes

We may update these Terms from time to time. The "Last Updated" date shows when they last changed. Continued use of the Site after changes means you accept them.
