Principal Illness Navigation (PIN)

Navigation for patients with a serious, high-risk condition—so they get through treatment, referrals, and scheduling without falling through the cracks.

What it is

Principal Illness Navigation is a Medicare service for patients with a serious, high-risk condition that is expected to last at least three months. It pays for the navigation work that helps patients move through their plan of care.

SageAlly's navigators do that work for your patients each month, document it, and send it to your provider for review and signature.

Services are billed by your practice and furnished by SageAlly's care team under your clinicians' supervision.

Which patients it's for

  • Cancer, including patients in active treatment
  • Chronic kidney disease and dialysis
  • Heart failure
  • COPD
  • Dementia
  • Other serious, high-risk conditions your clinicians identify

What our navigators do each month

  • Coordinate specialist referrals and confirm appointments
  • Follow up on prior authorizations
  • Help patients understand their plan of care and next steps
  • Schedule and track tests, imaging, and treatment visits
  • Share clinical-trial information where relevant
  • Arrange transportation to appointments
  • Connect patients to benefits and insurance support
  • Log time and activity for every patient interaction

What your practice does

  • Perform the initiating visit
  • Obtain and document patient consent
  • Provide general supervision
  • Review and sign monthly navigation notes
  • Submit claims under your NPI

Clinical questions are always routed back to your practice.

Documentation we provide

  • Time-tracked monthly notes per patient
  • A monthly sign-off packet for your provider
  • A billing summary for your biller
  • A monthly program report: patients served, time by program, issues escalated

PIN and CHI together

Many patients need both: PIN for navigating their illness and CHI for the social needs blocking their care. We can support both programs for the same patient, with time tracked separately per program, per CMS rules.
